Important: to be submitted with Payment Request Form



I  an officer, of   (company's legal name as stated in the contribution agreement), duly authorized on behalf of the organization, hereby represent and warrant:
  • that all payments received from Canada following receipt of claims, acceptable to NRCan under the terms and conditions of the contribution agreement, are for the reimbursement of eligible costs incurred and paid by (company's legal name as stated in the contribution agreement) and that all supporting documents to this effect have been kept in our records and will be made available to the Minister (NRCan) upon request;

  • that I have declared on the ecoENERGY for Renewable Heat program Payment Request Form any and all project funding received or anticipated to be receivable from government sources other than the ecoENERGY for Renewable Heat program; and

  • that the above noted declaration is true and accurate. I understand that if, in the opinion of the Minister (NRCan), there has been a misrepresentation or a breach of this warranty, the Minister (NRCan) could place the company in default of the terms, conditions or obligations of the contribution agreement, and may exercise his right to terminate the agreement and direct us to repay forthwith all or any parts of the monies paid by Canada pursuant to the agreement, and the amount will be a debt due to Canada and may be so recovered.
